An exciting opportunity exists for a Nursing Unit Manager Level 1 (NUM to join the Care Navigation and Care Coordination (CNCC) Team within the Keeping Well in Community (KWIC) Directorate. This leadership role is pivotal in delivering a nurse-led chronic disease management service that empowers patients to self-manage their health and reduces avoidable hospital presentations. The service operates predominantly via telehealth from an office-based environment, with home visits integrated based on clinical necessity. As NUM1, you will provide both operational and clinical leadership, guiding sound clinical decision-making to ensure interventions are person-centred, culturally safe, and aligned with service eligibility criteria. In addition to leadership responsibilities, you will maintain a clinical caseload, delivering comprehensive assessments, care planning, health coaching, and linking patients to appropriate health and social services. You will also oversee clinical governance for a dedicated nursing workforce, fostering professional development, monitoring care quality, and driving continuous improvement. Reporting to the Nurse Manager, KWIC, this role offers the opportunity to lead innovation in integrated care, collaborate across primary, community, and acute settings, and contribute to SWSLHD's vision of leading safe, sustainable care for healthier communities.

Selection Criteria

1. Relevant postgraduate qualifications and/or management experience or equivalent work experience or a combination of study and experience deemed appropriate by SWSLHD.
2. Describe your experience in providing operational leadership and managing a nursing team. How have you promoted a culture of excellence, safety, and continuous improvement while balancing workforce planning responsibilities such as rostering and performance management.
3. Explain how you have maintained an active clinical caseload while fulfilling leadership responsibilities. How did you ensure care remained aligned with best practice and patient-centred principles?
4. Share an example of how you have overseen clinical service delivery that supports chronic disease management and hospital avoidance. What strategies did you use to ensure holistic, culturally responsive care?
5. Discuss your experience in implementing clinical governance processes such as referral management, risk stratification, and case review. How have you used data to monitor performance and drive service improvement?
6. Applicants must possess an Australian C-Class driver's license (including P1 or P. Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
