Key Responsibilities
* Provide access and support participation in social, recreational, civic and vocational activities.
* Engage with individuals using a person-centred approach and ensure that people accessing services respect their goals, strengths and needs.
* Assist customers to develop skills and build capacity to actively participate in life at home or in the community.
* Maintain accurate customer records for individualised support.
* Build strong, professional relationships with customers, families, caregivers, and key stakeholders.
