Job Description:
We are seeking compassionate and motivated Disability Support Workers (Linkers) to join our dynamic team at Link to Life. As a Linker, you will provide 1:1 direct support to individuals with psychosocial disabilities, helping them navigate daily challenges and promoting their mental well-being. Your work will involve providing personalised care, assisting with daily activities, and encouraging social inclusion.
Key Responsibilities:
Provide high-quality, person-centred care and support to individuals with psychosocial disabilities
Assist participants with daily living activities such as personal care, meal preparation, and medication management
Support participants in engaging with their communities and participating in social and recreational activities
Implement individualised care plans and support strategies tailored to mental health needs
Foster a positive and supportive environment that encourages participant independence and well-being
Monitor and report on participant progress and any changes in their needs
Collaborate with other team members and healthcare professionals to ensure comprehensive care
Qualifications:
* Certificate III or IV in Disability, Mental Health, Individual Support, or equivalent (desirable but not mandatory)
* Previous experience in disability support, mental health, or a related field (preferred)
* Strong communication and interpersonal skills
* Empathy, patience, and a genuine passion for supporting individuals with psychosocial disabilities
* Ability to work both independently and as part of a team
* Lived experience of disability and/or mental health challenges (highly valued)
* Valid driver's license and access to reliable transportation
* Current First Aid and CPR certification (or willingness to obtain)
* Ability to obtain a NDIS Worker Screening Check and Working with Children Check
