Job Overview
We are seeking a highly skilled and compassionate Wellbeing Manager to join our team. As a Wellbeing Manager, you will be responsible for empowering clients to remain at the centre of their care journey by developing tailored Home Care Package plans and coordinating multidisciplinary support in their homes.

About the Role
* Develop and implement holistic care plans that address the physical, emotional, and social needs of clients.
* Coordinate multidisciplinary teams to provide high-quality care and support services to clients in their homes.
* Conduct regular assessments and reviews to ensure the effectiveness of care plans and make adjustments as needed.
* Provide clinical support and nursing care during scheduled appointments as part of the Wellbeing Manager role.
Requirements
* Certified registered nurse with current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
* Proven experience in aged care case management, ideally within community or aged care settings.
* Tertiary qualifications in Aged Care Case Management are highly regarded.
* Strong communication and interpersonal skills, with the ability to build relationships with clients, families, and healthcare professionals.
* A valid Australian driver's license and willingness to travel locally for client visits.
