Job Title: Hospice Care Specialist
In this role, you will provide care to patients in their own homes, working closely with a team of healthcare professionals to deliver high-quality support and assistance.
Your primary focus will be on improving the patient experience, helping them achieve their care goals, and promoting safe and early discharge from hospital settings.

Key Responsibilities:
* Provide personal care and support to patients in their homes
* Assist patients with daily living tasks, such as bathing, dressing, and medication management
* Work collaboratively with healthcare teams to develop and implement individualized care plans
* Support patients in achieving their care goals and best possible level of health and function
* Promote safe and early discharge from hospital settings through effective communication and coordination with healthcare teams

Required Skills and Qualifications:
* Allied Health Assistant Certificate III or IV or other appropriate qualification
* Previous experience working in hospital or community settings
* Computer literacy
* A current Victorian drivers licence
