We're seeking a dynamic and experienced Team Leader – Optimal Ageing to coordinate and drive high-quality, client-centered care across a multidisciplinary team of dedicated health professionals.
In this vital leadership role, you'll oversee the day-to-day delivery of community-based services, while also playing a key role in operational planning, service evaluation, and continuous quality improvement.
Your clinical expertise, combined with your leadership skills, will support a collaborative and responsive approach to ageing well in the community.
As a Team Leader – Optimal Ageing, you will: Provide strong operational leadership and coordinate a team of health professionals across diverse service models.
Monitor service delivery, lead data collection and evaluation, and ensure services align with community needs.
Contribute to service planning and integration across programs within the Community & Allied Health Directorate.
Foster workforce development, positive team culture, and high standards of care through coaching and collaboration.
Be accountable for quality outcomes, service innovation, and aligning clinical practice with strategic goals.
