About the Role
As our Neurological Liaison Nurse, you'll deliver high‐quality, person‐centred nursing care to clients living with a range of neurological conditions — including MS, Parkinson's, Huntington's, MND, ABI and Stroke.
Working autonomously but supported by a collaborative team, you'll manage a diverse caseload and provide clinical services in clients' homes, empowering them to remain independent and live well within their community.
Key Responsibilities
* Complete holistic nursing assessments and develop individualised care plans
* Provide direct clinical services (including continence, wound care, palliative support and catheter care)
* Work closely with MSWA's Allied Health teams to coordinate care
* Facilitate timely referrals to internal and external health services
* Travel throughout the community to support clients (pool vehicles available)
Must‐Haves
* AHPRA registration as a Registered Nurse
* Current First Aid and CPR certification
* NDIS Worker Screening Check (or willingness to obtain)
* Current C Class driver's licence
* Working with Children Check
Nice‐to‐Haves
* Experience in community or rehabilitation nursing
* Exposure to neurological conditions
* Knowledge of Disability Services Legislation and the NDIS
* Skills in wound care, continence, and palliative nursing
* Strong clinical judgement, problem‐solving skills and computer literacy
