McArthur is working with leading Not-for-Profit provider Uniting AgeWell, to find an experienced Occupational Therapist to join their team on a permanent basis. We are looking for caring, empathetic and committed staff, who can work autonomously, safely and confidently with Aged Care clients in North West Tasmania. What You'll Do:
In this role, you will work closely with the Team Leaders, Physiotherapists, Allied Health Assistants and PCAs to deliver person-centred, goal-directed care plans for clients referred through the My Aged Care program. Your responsibilities will include:
* Conducting home-based clinical assessments to evaluate clients' needs and functional abilities.
* Deliver OT services and therapy in home, centre or community environments
* Developing and reviewing care plans that focus on reablement and enhancing clients' independence.
* Arrange the provision of aids/equipment necessary for optimum function and independence of the client.
* Collaborating with clients and their families to set achievable goals and ensure their needs are met.
* Maintaining accurate clinical documentation in line with organisational standards.
What You'll Bring:
To thrive in this role, you should have:
* A Bachelor's degree in Occupational Therapy or an equivalent qualification.
* Registration with AHPRA.
* Happy to travel and use your own car
* Experience working with older adults, including those from diverse backgrounds and those living with dementia.
* Strong communication skills and the ability to engage clients holistically.
* Excellent organisational skills to manage your own caseload effectively.
* Be available for full days of work (Mon-Friday).
* An understanding of STRC/Post Acute Care/Transitional Care.
* Understanding of CHSP/NDIS funding and referrals.
