We have a wonderful opportunity for an experienced and motivated Clinical Care Coordinator to lead and mentor our dedicated team at our Residential Home in Croydon, Victoria.
The Clinical Care Manager plays a crucial role in coordinating person-centered clinical care for residents, adhering to established policies, procedures, and professional standards. Working in collaboration with the Quality Manager, the primary purpose is to concentrate on individual resident needs. This involves ensuring the existence of a current and well-documented Care Plan that effectively addresses residents' needs. This is a medium home, specialist provider that caters towards the needs of vulnerable people and those of low socio-economic means.

Your key responsibilities
* Assists with the admission of new residents to the facility, ensuring that they are assessed.
* Assess the individual needs of residents and develop person-centered care plans in consultation with other healthcare professionals, ensuring that care is tailored to meet their specific needs.
* Collaborate with other healthcare professionals to deliver comprehensive care to residents.
* Communicate effectively with residents' families and health care providers, keeping them informed about their health conditions, care plans, and any significant changes or concerns.
* Maintain accurate and up-to-date resident documentation, including care plans, behaviour charts, medication charts, observations and/or incidents.
* Provide and assist with direct nursing care to residents, including and not limited to assisting with activities of daily living (ADLs), administering and managing medication, catheterisation, conducting observations and monitoring, and wound management.
* Actively participates in case conferences and relevant meetings
* Complete clinical assessments and reviews at required intervals and escalate as required, including COD status, daily wound care and fall management
* Undertake audits as required, including the Schedule 8 drug count
* Collate data and prepare information for reports as required
Your skills & experience
* Bachelor of Nursing
* Current Australian Health Practitioner Regulation Agency (AHPRA) registration
* Nursing experience within a residential aged care facility, preferably at least 3 years of aged care experience.
* Experience of working cooperatively with multi-disciplinary care teams
* Strong clinical assessment, documentation and practice skills
* Effective communication and interpersonal skills
* Proficient computer and information management skills, including ability to use Microsoft and clinical software systems.
