People Who Care is a not-for-profit organisation with over 45 years of service supporting older people, people with disability, and individuals experiencing social crisis. We work in partnership with our community to deliver services that promote independence, dignity and wellbeing.
We are seeking an experienced and motivated Care Coordinator (Home Care) to join our team in this newly created role. This position plays a key role in coordinating high-quality Home Care services that enable clients to live independently and safely in their own homes.
You will work as part of a supportive and committed team, delivering person-centred, wellness-focused services in line with the Aged Care Quality Standards. This role is ideal for someone with strong organisational skills, excellent communication, and experience in the aged care or community services sector.
This role is based in Mandurah and offered on a permanent part time - 3 days per week.
Rate: $44.14 per hour base rate + Salary Packaging and Superannuation.
Responsibilities:
* Coordinate and administer Home Care services in line with the Aged Care Quality Standards
* Undertake service planning, case coordination, budget management and Consumer Directed Care administration
* Develop and review service delivery plans in collaboration with clients
* Monitor at-risk clients and regularly review services to ensure ongoing suitability
* Maintain accurate client records and service data
* Identify and resolve operational issues as they arise
* Uphold wellness, re-ablement and client-focused service delivery principles
* Ensure compliance with health and safety requirements
Essential Criteria:
* Certificate IV or higher in Aged Care, Disability, Individual Support, Community Services (or similar), and/or significant experience in in-home aged care at a coordination level
* Experience building collaborative partnerships with stakeholders
* Knowledge of relevant community resources and services
* Strong computer skills, including Microsoft Office
* Confidence using Client Record Management system
* Excellent written and verbal communication skills
* Strong organisational skills and the ability to manage competing priorities
* Ability to work effectively in a fast-paced environment
* Australian work rights
* Current Western Australian Driver's Licence
* Current National Police Clearance (issued within the last 6 months)
