Established in 1991, Danila Dilba HealthService (DDHS) is an Aboriginal community-controlled organisation committed toproviding culturally appropriate, comprehensive primary health care servicesand community development within the Biluru (Aboriginal) community of the YilliRreung (Greater Darwin) Region in the Northern Territory.

ABOUT THEROLE

The Population Health Team Leader willprovide leadership and support to service wide public health programs includingCapacity Building Portfolios. They will liaise with senior and clinic managersacross DDHS Integrated Clinical Services and Programs to support the deliveryof community education and promotional events across Danila Dilba HealthService.

WHAT YOU WILL DO

* Developthe DDHS Population Health Strategy including quality improvement within theclinics and community education
* Lead thedesign of community and staff consultations with the support of the AboriginalCultural Lead and Health Education and Engagement Lead
* Supportand provide leadership for service wide clinical education training across DDHS
* Supportthe Population Health team with the following quality improvement activitiesfor each portfolio within the Danila Dilba Clinics:
* Data Quality and Reports
* Clinic Flow
* Client Journey and Engagement
* Ensure aContinuous Quality Improvement approach is taken within identified PopulationHealth priorities
* Supportservice wide health promotion events, alongside communications team and Healtheducation and engagement team.
* Support the Health Education andEngagement lead in the coordination, monitoring, implementation and evaluationof the Tackling Indigenous Smoking program
* Provide expert advice, projectoversight and support for service wide public health portfolios such as theRheumatic Heart Disease and STI/BBV projects.
* Contributeto effective clinical governance arrangements that ensure high levels ofquality and safety.
* Thisrole will be required to work outside of standard business hours to supportcommunity events.

SELECTION CRITERIA

* Tertiaryqualification related to Public Health or Health Professional Qualification(including Aboriginal Health Practitioner)
* Minimum2 years' experience Public Health/Population Health Coordination
* Minimum2 years' leadership experience including people and contract management.
* Experiencein consulting with Aboriginal and Torres Strait Islander Community and peers
* Demonstratedability in use of program logic, planning, managing, implementing, andevaluating projects.
* Well-developedinterpersonal, facilitation and negotiation skills
* Demonstratedskills in supervising and supporting staff working in Health Promotion andcollaborating with internal and external stakeholders.
* Well-developedformal written and verbal communication skills along with an ability to utiliseappropriate data bases and other computer programs.
* Demonstratedknowledge of the Aboriginal Community Controlled primary health care sector andthe role of quality in primary health.
* Experience with communicare and/or workingexperience in Aboriginal Community Controlled Health Organisation
* Program evaluation and data collectionexperience.

* Willing to undergo a PoliceCheck.
* Ability and willingness toobtain a Working with Children Clearance (Ochre Card).
* Current driver's licence.
* Current CPR certificate orthe preparedness to gain one within 6 months of commencement.
* Fully vaccinated againstCOVID-19 or an exemption as listed on the Australian Immunisation Register(AIR) be submitted.
