Job Summary
We are seeking a dedicated and compassionate professional to join our team as an Intake Specialist. This permanent full-time role will be based in Sale, VIC, and will involve working collaboratively with a multidisciplinary team to provide support to vulnerable families.
Main Responsibilities:
* Manage a caseload of clients with diverse needs and backgrounds, delivering high-quality service planning and coordination.
* Screen and triage clients, identifying their individual needs and developing targeted interventions.
* Collaborate with the team to conduct risk assessments and develop integrated plans.
* Apply for brokerage on behalf of clients when necessary, ensuring access to essential services.
* Refer clients to relevant services within the broader system, providing them with information and ongoing support.
Key Requirements:
* A Bachelor of Social Work or equivalent qualification is mandatory, along with relevant experience in child, youth, and families welfare.
* Well-developed risk assessment and management skills are essential, as well as a sound understanding of the MARAM Framework and Best Interests Principles.
* The ideal candidate will have a valid and current Driver's License, enabling them to participate in home visits and other community-based activities.
