Job Overview:
As a dedicated professional, you will be responsible for delivering high-quality case management services to clients, prioritizing their needs and preferences. This involves coordinating all aspects of care, including administrative tasks and client service rostering.
Key Responsibilities:
* To provide person-centered and enablement-focused case management to clients.
* To be involved in the entire care coordination process, ensuring seamless delivery of client services.
* To manage a caseload of clients under approved funding, maintaining budgets and onboarding new clients while managing care requirements to maintain client independence.
Requirements:
* Experience in delivering quality, client-focused case management.
* Understanding of various funding sources available in the Aged Care sector.
* Tertiary qualifications in Community Services, Case Management, or related disciplines.
* High level of proficiency in computer systems, including data entry.
* Experience in developing care plans for complex clients.
* A current and valid Australian driver's license.
* A reliable, comprehensively insured vehicle.
* Current First Aid qualifications (or willingness to obtain before commencement).
