Job Responsibilities
* Work 40 hours per week with 8‐hour shifts.
* Perform reassessments and update the plan of care based on patient response.
* Collaborate with the care team to coordinate therapy plans and discharge recommendations.
* Educate patients and caregivers on exercises, safety strategies, and home programs as appropriate.
* Communicate functional changes and barriers to the care team to support care planning.
Qualifications
* Licensed Physical Therapist with a minimum of 2 years clinical experience.
