Dementia Support Australia is looking for passionate health and aged care professionals to join our growing team. The role is a Fixed Term Contract until April 2028 based in Daw Park Office, South Australia. As a Dementia Support Coach, you'll provide practical, relationship‐based support, modelling and coaching for clients, carers and care staff across a variety of settings.

Responsibilities
11. Provide on‐ground support and implement relationship‐based strategies for clients, carers and care staff.
12. Model and coach best‐practice approaches to help carers and aged care staff understand and support people living with dementia.
13. Implement recommendation plans with guidance from the Team Leader, Dementia Consultant and Senior Consultant.
14. Facilitate and support the trial and implementation of brokerage items for clients and carers.
15. Collaborate with peers to build capability across aged care teams and support quality, person‐centred care.
16. Strong organisational and time‐management skills in a busy work environment.
17. Ability to work independently and collaboratively.
18. A commitment to excellent customer service and constructive conflict resolution.
19. A current driver's licence and safe driving record.
20. Willingness to travel intrastate and interstate for up to five days at a time.
21. Flexibility to work varying hours based on client and carer needs.
Qualifications
22. Qualifications in a relevant field such as Allied Health Assistant, Diversional Therapy, EN, Aged Care, Cert III / IV in Individual Support or Leisure and Health (highly desirable).
23. Willingness to obtain criminal record check (short‐listed applicants will be asked to consent).
