Job Title: Clinical Nurse Specialist
About the Role:
The Community Specialist Palliative Care Home Team provides expert care and support to patients with life-limiting conditions across Cambridgeshire in a community setting. The team works closely with other care providers to manage symptoms, support medicine management, and facilitate access to other hospice services.
The successful candidate will be responsible for delivering high-quality, patient-centered care to individuals with complex needs. This will involve assessing, planning, implementing, and reviewing therapeutic interventions, as well as organizing and managing individual caseloads.
Responsibilities:
* Holistically assess patients' physical, psychological, and social needs
* Plan and implement programs of care tailored to individual needs
* Maintain regular communication with other care providers, such as GPs and District Nurses
* Collaborate with the community team and multi-disciplinary team to effectively communicate patient needs
* Provide specialist nursing advice, support, and education to other healthcare professionals and volunteers
Requirements:
* Registered General Nurse with a post-registration qualification in cancer/palliative care or long-term conditions (or willingness to work towards this)
* Experience delivering specialist palliative care in a multi-professional environment
* Experience of teaching and education
* Valid driving licence and access to a vehicle for work purposes
* Subject to an Enhanced DBS Check (Child and Adult Barred lists)
Person Specification:
* Registered General Nurse
* Post-graduate qualification or equivalent experience in Palliative and End-of-Life Care
* Evidence of continued professional development
* Advance clinical assessment skills
* Operational experience in delivery of specialist palliative care
* Extensive experience of teaching and education
* Experience in a range of settings (hospital and community)
