About the Role
The Dubbo Brain Injury Rehabilitation Program (BIRP) seeks a skilled Case Manager to join our dynamic multidisciplinary team. As a key member of our team, you will be responsible for ensuring the delivery of contemporary brain injury rehabilitation services to adults with acquired brain injuries in the Western NSW Local Health District.
Key Responsibilities
• Ensure the delivery of high-quality brain injury rehabilitation services in accordance with the principles and guidelines of the NSW BIRP.
• Provide case management and coordination services to individuals with acquired brain injuries, including assessments, goal setting, and service planning.
• Collaborate with a multidisciplinary team of clinicians to provide comprehensive care and support to clients.
• Develop and maintain strong relationships with community agencies and organizations to enhance service delivery and outcomes.
Requirements
• A degree in social work, psychology, or a related field is required.
• Proven experience in case management, preferably in a brain injury rehabilitation setting.
• Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
• Ability to travel within the Health District.
