About The Role
The Nurse Practitioner supports our current model of care where exacerbations of chronic conditions are minimised, prevented and/or managed in the community through responsive, integrated person‑centred care. People with specialist knowledge in Chronic Heart Failure and Chronic Respiratory Disease are encouraged to apply. The Nurse Practitioner will join a dedicated team of health professionals working on a project called SAFE‑HF. This project is aimed at reducing hospitalisations for patients diagnosed with heart failure (HF). The project will involve travel to local primary care practices within the health service catchment area.
This role is integral in the assessment and management of client's wellbeing and operates from a person‑centred care framework and aligns with the social model of health.
About You
You are a highly motivated, dynamic Nurse Practitioner with a passion for chronic disease who will provide support to the most vulnerable people in our Community with Chronic Heart Failure, Chronic Respiratory Disease.
Proven ability to work both independently and collaboratively.
You readily build strong relationships with stakeholders in the delivery of client centred care.
You have strong written and verbal communication skills.

What You Need
* Demonstrated recent experience as a Nurse Practitioner.
* Current AHPRA registration.
* Valid Driver's licence and good driving history.
* Previous Community Nursing experience.
