Residential care case manager

About the role

The Case Manager provides comprehensive, trauma‐informed case management to young people living in residential and lead tenant care programs. The role focuses on developing, implementing and reviewing individualised care plans in line with Looking After Children (LAC) and DHHS requirements, supporting young people to achieve goals across education, health, wellbeing and independence. Working collaboratively with residential care teams, families, volunteers and external stakeholders, the Case Manager coordinates services, ensures statutory compliance, and promotes positive outcomes, safety and community connection for young people, while contributing to a therapeutic, structured and supportive care environment.

Location: Footscray, VIC. This is a Permanent Full‐time (Fixed term 6 months) opportunity within our Out of Home Care Program. The position reports to the Coordinator Residential & LT Case Management.

Key Selection Criteria

To be successful in this role, you will have:

• A knowledge and understanding of Aboriginal culture and values and an awareness of the current issues faced by Aboriginal children, young people and their families.
• Appropriate tertiary qualifications.
• Sound professional knowledge and theory base.
• Extensive experience in adolescent welfare practice.
• Demonstrated professional case management skills, especially in crisis intervention and behaviour management.

You must have a:

• Valid and current Driver's Licence;
• Satisfactory Criminal History Check (paid for by MacKillop);
• Valid Employee Working with Children Check;
