The Social Worker/Psychologist position within the Assertive Case Management (ACM) program is to work as part of the multi- disciplinary team that provides intensive care coordination to support consumers with chronic and severe substance use issues.
The ACM program will target clients with chronic and severe drug and alcohol dependence who experience multiple and complex needs. The ACM will provide support to assist in the client's stabilisation and reintegration, and where relevant, assist in managing the client's underlying cognitive impairment.
The outcomes of the ACM program are to stabilise and reduce drug and alcohol use, reduce preventable hospital presentations for substance use issues, improve access to longer-term treatment and support for clients, improve general physical and mental health, nutrition, and social functioning, and reduce the risk of relapse through the establishment of prevention strategies.
The Social Worker/ Psychologist is expected to act as an appropriate and effective role model and promote culture and supporting practices that reflect the NSW Health CORE values of Collaboration, Openness, Respect and Empowerment through demonstrated behaviours and interactions with patients, clients, and employees.
