About your opportunity: Ensure assessment and interventions including discharge planning occur in line with models of care, service objectives and in consultation with the broader multidisciplinary team. Work collaboratively and cooperatively as a member of multidisciplinary teams and the Social Work Department. This position reports operationally and professionally to the Director of Social Work. About your role: Deliver evidence-based social work clinical services in accordance with the Australian Association of Social Workers (AASW) Code of Ethics; AASW professional practice standards; and relevant AASW Scope/s of Practice. Conduct comprehensive psychosocial assessments, develop appropriate intervention plans, and achieve outcomes, informed by social work theory and contemporary knowledge of social determinants of health; medical terminology; legislation; resources; services and policy requirements. Participate in the development of policies, procedures and strategic planning for the work unit that are consistent with the achievement of Social Work and the Cairns Hospital service delivery objectives. Ensure patients and other staff are treated fairly and with mutual respect and understanding, regardless of gender, cultural, religious, ethnic and linguistic backgrounds. Provide clinical support and education to less experienced staff, students and multidisciplinary team members, commensurate with experience.
