The Role As the Nurse Clinical Specialist (Preventable Chronic Conditions Educator), you will provide specialist clinical outreach care to remote clients and specialist support, advice, and education to PPHC staff with the specific focus on improving outcomes in early detection and management of preventable chronic conditions in aculturally sensitivemanner.You'll be accountable for providing sound professional advice and for assessment, planning, review, evaluation, and coordination of comprehensive client care and/or casemanagement.
About You To be successful in this role, you'll have:Current registration with Nursing and Midwifery Board of Australia or eligible forsame.High-level clinical knowledge and skills in Preventable Chronic Conditions or related field, supported by strong decision-making and problem- solving skills.Commitment to alearning culture through clinical guidance and mentoring.Ability to travel to remote communities by 4 WD or light aircraft asrequired.Hold acurrent NT driver's licence (or eligible to obtain) and be confident driving amanual vehicle in regional and remotesettings.
