**Job Description:**
We are seeking a compassionate and dedicated individual to join our team as a Support Worker. As a Support Worker, you will be responsible for providing high-quality care and support to our clients in their homes.
Your main responsibilities will include assisting with daily living activities such as personal care, medication monitoring, meal preparation, shopping & social support and cleaning. You will also be required to regularly liaise with our Care Coordinators to provide updates on client care needs and carefully follow client care plans.
