* Salary range: $128,169 to $137,918 p.a. (HP4)
* 4 to 6 weeks annual leave with 14%-17.5% loading

About your opportunity
Post hospitalisation aftercare services have been identified as a promising suicide prevention strategy, helping keep people at higher risk connected with support services and networks that promote safe living.
In this role you will work closely with the Universal Aftercare Service provider to coordinate care for consumers who require clinical support following hospital discharge. The role ensures that individuals experience coordinated, cohesive and appropriate care across both their clinical and non clinical recovery journey.
