**Job Title:** Aged Care Rapid Response Team Clinical Nurse Specialist
About the Role
This is an exciting opportunity to join our team as a Clinical Nurse Specialist, leading clinical practice development and service delivery. The successful candidate will be responsible for coordinating responses to Residential Aged Care Facilities.
The CNS2 role involves working closely with managers and senior clinicians to develop specialist practice within a nursing team. You will utilize advanced clinical skills with autonomy, focusing on hospital avoidance strategies for people aged 70 and over in Residential Aged Care Facilities and the community.
Key Responsibilities
* Provide senior clinical leadership and develop specialist practice within a nursing team.
* Utilize advanced clinical skills with autonomy, working closely with managers and senior clinicians.
Requirements
To be successful in this role, you will need:
1. Current RN registration with AHPRA, with relevant post-registration qualifications and a minimum of 3 years' experience in Aged Care and/or Community Nursing.
2. Experience in comprehensive assessment of older adults, with knowledge of aged care, rehabilitation, and community care services.
3. Strong communication skills, computer literacy, and willingness to learn electronic health systems.
4. Ability to work independently and collaboratively within a multidisciplinary team.
5. Experience in initiating quality improvements and evidence-based practice.
6. Current NSW Driver's Licence.
