About the Opportunity
As an experienced Case Manager, you will work as a collaborative member of our Aged Care Team, operating in line with organisational requirements and program guidelines.
You will develop and review care plans that enable consumers to achieve goals and enhance quality of life.
To be successful in this role, you will need:
* Relevant qualifications such as Bachelor of Nursing/Allied Health/Social Work or Enrolled Nurse/Diploma Community Services combined with Aged Care experience.
* Prior experience in planning, co-ordination and delivery of services to older people who live at home, including care assessments and care plan development and review.
* Demonstrated knowledge and understanding of Aged Care related reforms, legislation, compliance requirements, quality standards, and the My Aged Care portal.
* A knowledge of the aged care and community services sector, regional service system, and local providers.
* The ability to work respectfully and collaboratively with Aboriginal and Torres Strait Islander peoples, their families, communities, staff, and Aboriginal controlled organisations.
