We are currently recruiting a Psychologist or Mental Health Social Worker to provide care coordination for people with severe and complex mental illness managed by skilled health practitioners such as GP'S in the community. The Clinical Care Planner will work within a recovery oriented framework with the individual and key personnel to improve physical health of the individual.
**SELECTION CRITERIA**:
- Relevant Qualification as a Psychologist or Mental Health Social worker
- Current and Full registration with the appropriate registration authority
- Experience in working with adults with severe and complex mental health needs
- Experience in delivery of evidence based mental health interventions and assessments.
- The ability to obtain a National Police Clearance and Working with Children Check
