**Remuneration**: $63.91 - $65.21 per hour + Salary packaging
**Hours Per Week**: 24

**Where you'll be working**:
The Dementia Advisory Service CNC position is a vital part of a small clinical team known as the Dementia Advisory Service which comprises one of the teams within the CACs GNS Aged Care Services. Other teams within CACs GNS Aged Care Services with whom the Dementia Advisory Service works closely include The Home Support Assessment Service, the ACAT Team, Community Dementia Nurses, Neuropsychology Service, and Hunter Carer Support.

**About the role**:

- Promote local community awareness of dementia, available services
- Provide information to people with dementia and their carers
- Lead local dementia carer support groups
- Provide dementia education to various stakeholders
- Support is provided by CACS GNS Aged Care Services Manager and CNC in Aged Care

**Requirements**:

- Registered Nurse with current Authority to Practice with AHPRA
- At least 5 years full-time equivalent post-registration experience and approved post-registration nursing/midwifery qualifications relevant to the specialty field, or such other qualifications or experience deemed appropriate by the public hospital or public health organisation

- Ability to prioritise workload within a small team
- Possess excellent written and verbal communication skills e.g. presentation skills, working with diverse groups
- Demonstrate professionalism in an empathic and caring manner
- Knowledge and understanding of my aged care and referral pathways for appropriate services
- Understanding of dementia and ability to provide comprehensive assessment and consultation for people with dementia, their families, and carers
- Understanding and provision of person-centred care in collaboration with clients, carers, and service providers to achieve identified outcomes
