Job Description: Be the vital link in Aboriginal health, delivering culturally safe care and supporting communities to improve health outcomes across settings.

This role involves working across clinical and community settings, providing wellness checks, case coordination, and culturally safe care delivery to Aboriginal clients aged 17 and over. The successful applicant will work effectively with multidisciplinary teams, ensuring holistic, culturally safe, and responsive healthcare delivery.

The role requires the ability to plan, develop, implement, and review Aboriginal health programs, including health promotion, education, and disease prevention initiatives that are culturally appropriate and community-driven.

In addition, the successful applicant will apply their knowledge of local Aboriginal communities to ensure healthcare delivery is holistic, culturally safe, and responsive. They will also maintain accurate documentation, data collection, and record management in line with policies and procedures.

This is an opportunity for a committed Aboriginal Health Worker/Practitioner to make a real difference in Aboriginal health and wellbeing, while developing skills, receiving supervision, and growing professionally.

Key Responsibilities:

* Deliver effective, safe practice within your assessed level of competence.
* Provide primary healthcare in partnership with the multidisciplinary team.
* Plan, develop, implement, and review Aboriginal health programs.
* Apply your knowledge of local Aboriginal communities.
* Ensure accurate documentation, data collection, and record management.

Requirements:

* Willingness to work towards obtaining a Certificate IV Aboriginal Primary Health Care (Practice) within two years of enrolment in course and commencement of employment.
* Current NSW Driver's Licence and ability to travel within the local area and throughout Hunter New England NSW LHD.
