Chronic & Complex Supportive & Palliative Care Clinical Nurse Specialist

Employment Type: Temporary Full Time (up to 02/10/2026)

Position Classification: Clinical Nurse Specialist Grade 2

Remuneration: $58.82 – $60.75 per hour

Hours Per Week: 38

Location: Auburn Community Health Centre

Where you'll be working: This position is based out of the Auburn Community Health Centre but will provide support as part of a wider team to the WSLHD Community Palliative Care Service. There is a requirement to attend occasional 10am-6:30pm shifts worked out of the Blacktown Community Health Centre. You'll be part of a supportive and collaborative multidisciplinary environment committed to providing high-quality health services to people living with chronic and complex conditions.

What You'll Be Doing

As a Clinical Nurse Specialist, you will provide advanced, person-centred nursing care to clients requiring chronic, complex supportive and palliative care. You will use your clinical expertise to assess, plan, implement and evaluate care, while contributing to service development, clinical leadership and continuous improvement.


Your Key Responsibilities

* Delivering safe, compassionate and evidence-based care tailored to individual client needs
* Leading developments in specialty nursing practice and care delivery
* Coordinating and participating in quality improvement and clinical audit activities
* Providing mentorship, guidance and expert clinical support to nursing and allied health staff
* Educating clients, carers and colleagues, adapting information to meet diverse needs


Essential Criteria

* Registration: Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA)


Selection Criteria

* A minimum of three (3) years full-time equivalent experience working in Palliative Care and/or End of Life care Nursing. Relevant post graduate qualifications preferred.
* Demonstrated skills and knowledge in case management, including care coordination for people living with a life limiting illness, using advanced clinical skills in patient/client assessment and care planning.
* Demonstrated ability to provide leadership in the provision and development of clinical practice using evidence-based research, demonstrated understanding of current NSW Policies, National and Standards related to Palliative Care.
* Demonstrated ability to communicate effectively with clients, carers and a diverse range of health care providers including those in government, non-government and community organisations.
* Demonstrated ability to work within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion with an ability to identify and improve opportunities to enhance continuity of care between the acute sector, primary care providers and community-based services.
* Demonstrated ability to work with consumers, families and carers in a time of high levels of family stress.
* Demonstrated ability to utilise information technology within a variety of applications and willingness to undertake training.
* Current Drivers Licence valid within Australia and willingness to use for work purposes.
