Support Worker - Port Macquarie, Mainsail

Join the New Horizons team as a Support Worker and work within our Program in the Port Macquarie region. 
This role provides support and works in partnership with people living with disability to live within the community in the way that they want to.

Key Responsibilities 
- Permanent Part time – between 48 to 64 hours per fortnight as a Support Worker to work within the Port Macquarie Program. 
- Collaborate with customers to develop individualised, person-centered support plans that incorporate identified wellbeing needs, strengths, and goals. 
- Provide support to customers to improve independence by assisting with skill development, transport, social activities, finances, appointments, cooking, medication and personal care, and other required support. 
- Support and encourage customers to participate in activities of their choice (social, leisure, sporting, educational, employment, etc.). 
- Maintain customer records daily using online systems and electronic databases. 
- Develop and maintain professional relationships with customers, families, caregivers and key stakeholders. 
- Assist customers to navigate the sector to access any additional services required. 
- Seek innovative ways to enhance the recovery journey and overall customer experience with New Horizons.

What you will bring to the role 
- Minimum Cert III in Disability, Mental Health, Peer Work, or equivalent. 
- Valid NSW Driver’s License (Class C) with ability to drive large cars and/or vans. 
- Willingness to obtain an Australian Federal Police Check, NSW Working with Children Check (WWCC) and NDIS Worker Check prior to commencing with us. 
- Strong values guiding your work, incorporating our values of passion, integrity & respect. 
- Good computer skills with proven ability to use Microsoft Office and customer relationship management systems. 
Desirable 
- Experience in disabilities or mental health conditions. 
- Advanced knowledge of services and programs under NDIS. 
- Understanding of Person-Centered Practices.
