Our organization is seeking a dedicated and experienced Senior Social Worker to join our dynamic Transitional Care Team.

This is a recruitment pool for Permanent Full-Time/Part-Time, Fixed-Term Full-Time/Part-Time, and Casual appointments with the possibility of extensions and/or permanency.
Suitable applicants will be placed in a pool from which appointments may be made when similar vacancies occur.
Required Skills and Qualifications
* Minimum 3 years post-qualifying experience (AASW)
* Excellent relationship-building abilities
* An ability to work independently and collaboratively
* Passion for providing exceptional care to our clients and their families/significant others
