This is a rewarding position to support individuals transitioning from hospital settings back into their homes or residential care.
We are seeking an experienced professional who can apply their expertise in assessment and biopsychosocial approaches, implement practical strategies to prevent, minimise or respond to changes in behaviour related to dementia.
About the Role
* Support families, caregivers and clinical staff to understand how dementia impacts each individual, providing guidance on tailored support plans.
* Equip carers and clinicians to prevent and respond to behaviour changes through coaching, mentoring, and development of resources including behaviour support planning.
* Develop contextualised reports that drive positive change, improving quality of life for people living with dementia.
* Undertake holistic assessments to gain a deeper understanding of the individual's experience and care network.
Key Skills and Qualifications
* Demonstrated expertise in supporting people living with dementia.
* Experience in Australian residential aged care and/or community aged care.
* High level of negotiation, communication, and relationship building skills.
* Ability to prioritise multiple tasks and meet deadlines.
