What you'll be doing
The Nurse Practitioner/ Transitional Nurse Practitioner Aged Care will function autonomously and collaboratively to provide a high level of clinically focused nursing care, practicing within their regulated scope of practice, across diverse practice settings and patient/client populations within a defined specialty.
The-NP will contribute to the advancement of quality health care systems within the Local Health District and across Residential Aged Care Facilities (RACF's) to reduce avoidable hospital presentations and promote early discharge from hospital.
To manage care across a variety of health systems to maximize outcomes - engaging in complex and critical thinking; integrating information and/or evidence; judiciously use clinical investigations; skilfully and empathetically communicating with all involved in the care episode, including the person receiving care and their family and community, and health professional colleagues.
To engage in practice as guided by the Nursing and Midwifery Board of Australia Nurse Practitioner Standards Framework.
About you
Do you have current registration with the Nursing and Midwifery Board of Australia with endorsement to practice as a Nurse Practitioner or currently working toward endorsement as a Nurse Practitioner and ability to complete requirements within a 3-year period and At least 3 years full time equivalent post registration nursing experience with extensive clinical experience in aged care/psychogeriatrics and ability to function at an advanced autonomous level.
