* Monitor clients' health and well-being, reporting any concerns or changes to our management team.
* Work collaboratively with our multidisciplinary team to ensure seamless care coordination and communication.
* Participate in ongoing training and professional development to enhance your skills and knowledge.
Requirements
* Degree in Social Work, Nursing, or a related field.
* Minimum 2 years of experience in aged care or a related field.
* Excellent communication and interpersonal skills.
* Able to work independently and as part of a team.
* National Police Clearance and current Australian Driver's Licence (minimum P2).
