Support Workers Needed
We are seeking dedicated and compassionate support workers to provide assistance for young people participating in our early intervention program.
Job Description:
The role of the support worker is to deliver high-quality services to young people, conduct assessments and develop care plans, and provide ongoing support and guidance. This will involve working closely with the team to implement care plans, maintaining accurate records, and liaising with referrers and service providers.
Responsibilities:
* Deliver case management services to young people in the program
* Conduct assessments and develop care plans
* Provide ongoing support and guidance to clients
* Maintain accurate records and statistics
* Liaise with referrers and service providers
Qualifications and Skills:
* Experience working with young people aged 12-25
* Minimum 2 years direct casework experience
* Excellent communication and interpersonal skills
* Ability to work effectively as a team member and practitioner
* Strong personal boundaries and commitment to culturally safe practice
