About The Role
This Senior Community Mental Health Professional is located within the Croydon Assertive Outreach team. It is responsible for providing advanced evidence-based, strengths focused, recovery‐orientated assessment and treatment within a multi-disciplinary team and coordinating consumers' recovery plans where there have often been poor treatment outcomes in the past. The senior role is also responsible for complex clinical decision making, supervision and consultancy to colleagues, Community Managed Organisations, GPs and our community. This will include advanced reasoning skills in community mental health care implementing advanced practise skills with the complex care consumers of the Assertive Outreach team who often have experienced long periods of institutional care.

Ideal Candidate
* Relevant qualifications recognised with the Occupational Therapy Board of Australia and the Australian Health Practitioner Regulation Agency (AHPRA) or minimum bachelor degree in Social Work eligible for membership of the Australian Association of Social Workers (AASW). Must possess a current NSW Drivers Licence (P1 and P2 acceptable).
* Demonstrated advanced knowledge of and skills in recovery oriented services and consumer involvement in service design, implementation and evaluation.
* Demonstrated advanced knowledge and skills in the comprehensive assessment and management of complex mental health consumers in the community, including assessment, complex care coordination, and discharge planning.
