About the Role
Community Care Workers are part of a broader team working with the client to optimise their health and independence. Our Home and Community Care Program follows the Active Service Model (ASM) principles and is part of a wider initiative to build capacity in Victorian Home and Community Care services and make them more person centred. The approach focuses on people's strengths, not their deficits, and assumes everyone has some capacity to improve their own health and wellbeing.
The successful applicants will provide a valued and key role in delivering high quality, professional and integrated support services for people living in the community. You will be responsible for:
* Ongoing support services in the community for people who require assistance with self-care and domestic duties.
* Assisting people to access the community.
* Actively support clients to work towards their goals.
Applicants will be expected to work flexible shifts. This will include split shifts, evenings and weekends. Applicants should have sound communication and organisational skills, a current driver's license along with a fully insured, roadworthy vehicle.
About You
The successful applicant should possess the following:
* Certificate III in Individual Support (Home and Community).
* Current Police Check, Working With Children Check and NDIS Worker Screening Check.
* Current First Aid Level 2 Certificate.
* Valid Driver's License and reliable vehicle with fully comprehensive insurance.
Staff have access to supportive staff mentoring, education and development programs. Salary Packaging is available for all staff. The successful applicant will be required to provide a current satisfactory police check, Working With Children Check and NDIS Worker Screening Check.
