STAR Community Services is an award-winning not-for-profit supporting elderly and people with disabilities since 1996. We provide affordable, reliable aged care and disability services—including Support at Home, NDIS, transport, social activities, and companionship—to help clients live independently and stay connected. With help from dedicated staff and local volunteers, we serve Redlands, Ipswich, Logan, Gold Coast, and Toowoomba. STAR is committed to building inclusive communities and combating social isolation through regular programs and events.
About the Role
As a Community Registered Nurse, you will:
· Conduct comprehensive in-home clinical assessments
· Develop and implement personalised care plans
· Provide wound care, medication administration, chronic disease support, and end-of-life care within scope
· Collaborate with Care Partners and other team members to ensure holistic, culturally safe care
· Educate and support care staff, Participants, and families
· Contribute to service excellence through quality improvement and compliance with the Aged Care Act 2024 and Strengthened Aged Care Quality Standards
This is a dynamic, independent role suited to a nurse who thrives in a mobile, community-based environment and wants to contribute meaningfully to participant wellbeing and dignity.
Key responsibilities (not limited to)
· Deliver safe, evidence-based clinical nursing interventions in participants' homes
· Complete accurate and timely documentation in line with aged care standards
· Supervise and delegate tasks to Care Partners and Direct Support Workers
· Build strong, respectful relationships with participants and their care networks
· Contribute to audits, clinical reviews, incident reporting, and continuous improvement
· Actively engage in professional development and maintain AHPRA registration

What will you bring to the role?
Essential:
· Bachelor of Nursing + current AHPRA registration as a Registered Nurse
· Experience in aged care, community, or home-based nursing
· Strong assessment and care planning skills
· Knowledge of aged care legislation and person-centred care models
· Ability to work independently, manage time, and prioritise care
· Intermediate computer and mobile technology skills
· Alignment with STAR's values of inclusion, empowerment, respect, and compassion
Desirable:
· Postgraduate qualifications in gerontology, wound care, palliative care, or community health
· Understanding of trauma-informed and inclusive care approaches
· Experience supporting people with complex needs, dementia, or chronic illness
