The Team: Royal Hobart Hospital is Tasmania’s largest hospital, and the Adolescent Unit, as part of Paediatric Services, within the RHH provides care to young people aged 12-17. The Adolescent Unit admits patients across a range of specialties, including medical, surgical, and mental health. The Adolescent Unit features two dedicated Mental Health Observation Suites to accommodate mental health consumers. Paediatric Services collaborates closely with Child and Youth Mental Health Services, Allied Health, Child Safety Liaison Officers, and other services to deliver holistic care to Tasmanian adolescents. The Role: The Integrated Care Coordinator is a direct care team member of Paediatric Services, based out of the Adolescent Unit at the Royal Hobart Hospital. The Integrated Care Coordinator will: Provide subject matter expertise on complex behaviours and the Mental Health Act in relation to children and adolescents. Develop and execute care plans, coordinate resources, and lead discharge planning. Provide leadership to the Paediatric nursing team related to behaviourally complex or mental health consumers, including leading de-escalation efforts and hot and cold team debriefs following incidents. Champion unit initiatives, including de-escalation frameworks and education, hot and cold debrief models, and staff wellbeing programs. Collaborate with and act as a primary liaison between clinical stakeholders including Child and Youth Mental Health Services, Paediatric Medicine, Child Safety Liaison Officers, Adult Mental Health Services and Allied Health Services. Provide consultation where required by other roles, units and services. Develop and maintain policies and procedures related to child and adolescent behaviourally complex or mental health consumers in partnership with relevant stakeholders. Provide routine education to Paediatric Services staff on best practices related to complex or mental health consumers. You will need: Demonstrated high level skills in the clinical coordination of patient care, patient flow principles and discharge planning. Demonstrated ability to lead and/or work effectively with an inter-professional team in the planning and coordination of clinical care for patients requiring specialist care using problem solving skills at an advanced level. Demonstrated advanced interpersonal skills including written and verbal communication together with a proven ability to develop and maintain effective working relationships as well as the ability to prevent and manage conflict. Details of appointment: Permanent, full time, day worker position, working 76 hours per fortnight, commencing as soon as possible. *notwithstanding hours to be negotiated with the successful applicant. Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies. Salary: $109,767 to $115,813 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount. Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.
