Clinical Care Lead Position
We are seeking a highly skilled and motivated Clinical Care Coordinator to join our dedicated team in the Northern Rivers region. This pivotal leadership role combines clinical nursing, care coordination, and relationship management to support elderly, frail, and disabled clients with person-centred, high-quality care.
Key Responsibilities:
* Conducting clinical assessments and developing tailored care plans
* Overseeing the coordination of an allocated case load of client services and supporting care staff
* Providing direct nursing care when required
* Identifying and managing clinical risks with a proactive approach
* Offering training and support to team members
Client-Focused Care:
* Conduct in-home assessments and prepare individualised care plans
* Provide clinical education and support to clients and their families
* Monitor care quality and implement nursing interventions as needed
Requirements:
* AHPRA Registration as a Registered Nurse (essential)
* Demonstrated leadership and coordination experience in aged or community care
* Strong knowledge of aged care legislation, WHS, and quality standards an advantage
* Excellent interpersonal, problem-solving, and documentation skills
