About the RoleWe are looking for a dedicated Support at Home Care Partner to provide exceptional care and services that enable older individuals to remain safely at home. You will play a vital role in developing, implementing, and monitoring personalized support plans. This role is about one hour from Warrnambool.
Key ResponsibilitiesWork within a legislative framework to ensure compliance with the Support at Home Program.
Guide Health Care Workers to deliver safe and timely care services.
Actively identify and manage safety risks for participants.
Collaborate effectively within a team and ensure high-quality service delivery.
Communicate openly and participate in multi-disciplinary team meetings.
Minimum QualificationsCare Management Qualification (or currently working towards)
Cert 4 Individual Support- with relevent expereince
Current registration with the Nursing and Midwifery Board of Australia or similar
Valid Victorian Driver's License
Current Police Check
Current AHPRA registration (if required)
Influenza Vaccine (Current Year)
Key Selection CriteriaEssential:
Strong time management and prioritization skills.
Experience in individualized support within a person-centered framework.
Ability to identify and manage participant safety risks effectively.
Desirable:
Familiarity with software such as e-Tools, TRAKCare, RiskMan, and Microsoft Excel.
