Clinical Lead Community Care (Registered Nurse)
Pennwood Aged Care Services is seeking a clinically strong, values-aligned, and forward-thinking Registered Nurse to step into the role of Clinical Lead – Community Care Services.
This role offers a unique opportunity to influence and lead high-quality, community-based aged care services across Support at Home, Home Care Packages, CHSP, and community-based TCP programs, all from our Pennington-based Head Office.
The Role
Reporting to the Head of Community Care Services, the Clinical Lead is a senior clinical position responsible for:
Providing leadership and clinical oversight to a multidisciplinary team of Care Managers (including RNs, ENs, and non-clinicians)
Driving the delivery of safe, effective, and person-centred care across all community programs
Supporting complex client care planning, reassessment, and clinical escalation processes
Monitoring and responding to clinical risks, incidents, and compliance requirements
Ensuring adherence to the Aged Care Quality Standards, the Aged Care Act, and the Support at Home reforms (commencing November 2025)
Supporting onboarding, mentoring, and professional development of frontline care staff
Acting as a key clinical interface with external providers, allied health, and regulatory bodies
What We're Looking For
Current registration as a Registered Nurse Division 1 with AHPRA
Demonstrated experience in aged care, community care, or home care
Proven clinical leadership capability – including staff supervision, escalation of risk, and multidisciplinary collaboration
Strong understanding of care planning, clinical governance, and regulatory frameworks
Excellent verbal and written communication skills
Comfortable working in digital care systems (experience with eCase or similar is a bonus)
Ability to work independently while contributing to a collaborative, values-driven team
