The role of the Mental Health and Housing Liaison Officer (MH&HLO) is to link people who require mental health services with housing services, focusing on those who are homeless or at risk of homelessness. This involves improving connections and pathways between mental health services and housing agencies to achieve better outcomes for consumers.
As a MH&HLO, you will utilise your expertise in mental health and housing models to support complex care coordination, ensuring community tenure and access to public and social housing. You will also maintain tenancies through support packages such as HASI and CLS.
You will work closely with key mental health teams, housing teams, and community managed organisations to provide project management and consultancy services. Additionally, you will be part of local committees, collaborating with health, housing, and other partner agencies in the district.
This role requires mobility throughout the district, including provision of in-reach to inpatient services and co-location at housing offices.
