Job Summary
This role is an opportunity to partner with customers in a community setting, providing individualized care that connects them to the people, passions, and places they love.
The successful applicant will be responsible for conducting clinical nursing assessments, developing and implementing plans of care, and liaising with healthcare professionals. They will also participate in multidisciplinary team meetings to ensure seamless care delivery.

About the Role
This is a fixed-term contract until December 2026, working 30 hours per week: Monday, Tuesday, Thursday, and Friday. The position involves travel across Perth central metropolitan areas.

Requirements
To be successful in this role, you will require:
* Current registration with AHPRA as a Registered Nurse
* Ability to relate to elderly customers with empathy and understanding
* Strong clinical assessment skills and ability to implement effective care plans
* Autonomy and enthusiasm for professional growth and development
