Support Clinician Role
We are seeking a highly skilled and compassionate Support Clinician to join our multidisciplinary team in providing high-quality care and support to young people.
The Uplift Youth Support Program is a youth-friendly service that provides age-appropriate services engaging and acceptable to young people. As a clinician on this team, you will work collaboratively with other professionals to deliver evidence-based clinical care and case management.
Key Responsibilities:
* Deliver high-quality clinical care and case management as part of a multidisciplinary team
* SUPPORT THE DELIVERY OF MENTAL HEALTH SERVICES ACROSS COMMUNITY AND INPATIENT SETTINGS
* Conduct biopsychosocial assessments, referrals, crisis management, and discharge planning according to professional standards
Requirements:
* Full registration as an Occupational Therapist, Nurse, or General/Provisional registration as a Psychologist/Clinical Psychologist, OR AASW registration or eligibility as a Social Worker
* Excellent communication skills and ability to work in a team environment
* Prior experience in mental health case management (desirable)
