Your Role
* Conduct comprehensive assessments across a diverse, manageable caseload
* Develop tailored treatment plans that support mobility, independence, and wellbeing
* Collaborate with RNs, Exercise Physiologists, Care Managers, and other Allied Health colleagues
* A blend of community visits and working from home
* The autonomy you want, with the backup you need
