Where You'll Be Working

Based at 621 Hunter Street, Newcastle, the HNEKids Transition Care Service is part of Youth Health Services and provides vital coordination support to young people aged 14–25 with chronic illness or disabilities. The service assists in the transition from paediatric to adult health care systems, improving long-term engagement and outcomes.

You'll be part of a close-knit, collaborative team that values open communication, mutual respect, and a shared commitment to supporting young people and their families. Enjoy a positive workplace culture with a strong focus on professional growth and work-life balance.

About The Role

As Transition Care Coordinator, you'll support young people through a critical stage in their healthcare journey. Your role includes coordinating individualised care plans, working directly with young people, families, and healthcare providers. You'll attend paediatric and adult health clinics, support the development of self-management skills, and follow up post-transition to ensure continued engagement in care.

This is a dynamic, person-centred role with a focus on respect, collaboration, and empowerment. You'll be supported by the Senior Transition Care Coordinator and the Manager of Youth Health Services – Children, Young People and Families.

About You

You are empathetic, patient, and able to build trusting relationships with young people and their families. Your strong communication and organisational skills enable you to effectively navigate complex care needs while managing competing priorities.

You thrive in a team environment, are proactive in your learning, and are committed to delivering coordinated, high-quality care. Familiarity with healthcare systems is desirable, but not essential—training and mentorship will be provided.
