Experienced Social Worker required for a large hospital in North Queensland. You will join the long stay team of Social Workers and Occupational Therapists.
The role involves:
1. Coordinating residential aged care activation
2. Managing an interim facility caseload of subacute / maintenance patients who have left hospital and are temporarily staying in QLD Health funded aged care beds while awaiting permanent placement to RACH.
3. Providing case management, advice, and preventing hospital admissions.
With variable experience acceptable, the ideal candidate will have knowledge of the aged care system and some exposure to working with older people, disability, or the NDIS.
Achievable Objectives
1. To coordinate effective residential aged care activation strategies.
2. To manage and maintain a high-quality caseload of subacute/maintenance patients.
3. To provide expert case management, guidance, and preventative measures to reduce hospital readmissions.
Additional Requirements
* Experienced HP3 or HP4 clinicians welcome to apply.
* A flexible start date is available, potentially leading to an extension of up to six weeks on the wards.

* If trained overseas, applicants must have been assessed as eligible for AASW membership.
* Relevant experience is essential.
