We're seeking a compassionate and skilled Clinical Care Coordinator in Gympie, to manage a clinical caseload, lead risk assessments, and coordinate holistic, person‐centred care for clients with severe mental health conditions across the CCQ region. Working closely with a multidisciplinary team, you'll support smooth transitions between services, drive quality improvement, and contribute to positive client outcomes through ethical, collaborative practice.
Responsibilities
* Conducting clinical assessments and developing tailored treatment plans
* Deliver evidence‐based psychosocial interventions and service improvement
* Managing caseload and conducting risk assessment with management plans
* Collect data and supporting monitoring and evaluation
* Collaborate with teams and external stakeholders
* Facilitate care planning and review processes
* Monitor and escalate mental health needs as appropriate

Requirements
* Mental Health Accreditation
* Current AHPRA practicing certificate or AASW registration
* Tertiary qualification in psychology, social work, mental health nursing, or similar
* Experience managing complex mental health cases and trauma‐informed care
* Experience working with diverse cultures, especially First Nations people
* Strong teamwork and collaboration skills, problem‐solving and organisational abilities
